We are pleased to inform all active UTLA members that UTLA provides an automatic $1.000.00 accidental death and dismemberment coverage.  In addition, we are happy to introduce you to the UTLA approved Hartford’s Supplemental Accidental Death Plan that can help provide your family with a Guaranteed Income at a surprisingly affordable cost.  Accidental death can happen to any employee, even those employees who are in their peak earning years, just at a time of high financial obligations (loans, mortgages, and children to put through school) and low savings.  We feel so strongly about this insurance program that we are providing all active members in good standing with $1,000.00 in accidental death and dismemberment coverage.

You now have the opportunity to purchase supplemental amounts of accidental death coverage with a Guaranteed Income, up to $1,000.00 a month. That $1,000.00 a month can help pay the rent or mortgage, provide college education, build a trust fund for children or grandchildren, or simply mean the difference between your family living comfortably or just living.  Your family depends on your income now.  Don’t deny them the same should they face the future without you.  Please refer to the accompanying brochure for costs exclusions, limitations and terms of coverage.  Benefit amount will reduce at age 70 according to the schedule in your certificate.

How to assure your $1,000.00 automatic coverage:  Even if you do not wish to take advantage of the affordable prices available on the supplemental amounts of accident insurance, you should still complete the enrollment form for the automatic $1,000.00 coverage.  This allows the company to record for their files your beneficiary designation.  If you wish to purchase the supplemental coverage that provides the guaranteed income, through payroll deduction, please indicate your coverage choice.  Please give this program your most serious consideration.
